Creek Youth Sports is thrilled to announce two one-day Girls Flag Clinics on Saturday, December 7 and Saturday, December 14, exclusively for girls in 4th - 8th grade.
These faith-based clinics will provide young athletes with the perfect opportunity to learn the fundamentals of flag football, gain new skills, and experience the excitement of the sport in a supportive and inclusive faith based environment.

Coaches: Varsity girls from Berean Christian’s Flag Football team
These clinics are designed to introduce girls to the sport of flag football, helping them prepare for middle school and high school level play. Led by the Berean Christian Varsity Girls Flag Football team, alongside some phenomenal coaches within the field of Flag Football all, all participants will have the unique opportunity to learn from experienced players & coaches, in a faith-based setting that encourages teamwork, sportsmanship, and leadership.
Whether your daughter is new to the sport or looking to enhance her skills, these clinics will provide a foundation for success both on and off the field

Acknowledgment and Assumption of Risk
I, the undersigned, in consideration of my child's participation in the Creek Youth Sports Co-Ed Flag Football League, acknowledge the physical and general risks involved in such athletic activities. I am aware that sports activities such as flag football involve potential risks of injury, including but not limited to falls, contact with other participants, and conditions of the playing field. I hereby assert that my child is physically fit and mentally capable of participating in flag football activities. I agree to assume all the risks and responsibilities surrounding my child’s participation in these activities.
Waiver and Release of Liability
By signing this form, I, as the parent/guardian of the above-named participant, waive, release, and discharge Creek Youth Sports, its staff, volunteers, and representatives from any and all liability for injury, death, loss, or damage arising out of my child's participation in the league. This release includes any claims based on negligence, actions, or inactions of the above-named organization and its representatives.
Indemnification
I agree to indemnify and hold harmless Creek Youth Sports and all affiliated parties from any claims, demands, losses, or damages, including reasonable attorneys’ fees, arising out of or related to my child's participation in the league, including any injury or damage resulting from the same.
Medical Treatment Authorization
In the event of an injury to my child during league activities, I give permission for Creek Youth Sports representatives to seek and obtain medical treatment for my child if I am not immediately available to give authorization. I am aware that every effort will be made to contact me to obtain my specific authorization before any treatment is given.
Media Release
I grant Creek Youth Sports the right to photograph or video my child and to use the photo, video, or film likenesses for any legal use, including but not limited to publicity, illustration, advertising, and Web content.
Parent/Guardian Agreement
I have carefully read this waiver and release form and fully understand its contents. I am aware that this is a release of liability and a contract between myself and Creek Youth Sports.
